How HEAL collects, uses, stores, and safeguards information across the website, services, facility, and STYKU scan experience.

HEAL Wellness LLC, doing business as HEAL Wellness + Longevity ("HEAL," "we," "our," or "us") is committed to protecting your privacy. This Privacy Policy explains how we collect, use, store, and safeguard your information when you visit our website, use our services, or visit our facility. By using our website or services, you agree to the terms outlined in this Privacy Policy. If you do not agree, please do not use our site or services.

1. Personal Information We Collect

We may collect the following categories of personal information:

  • Name (first and last)
  • Email address
  • Mailing or billing address
  • Phone number
  • Payment details (processed through PCI-compliant payment gateways; HEAL does not store full card numbers)
  • Date of birth or age verification (where applicable)
  • Health and wellness preferences you choose to share with us
  • Emergency contact (where provided)
  • Visit history, membership status, credit balance, and pack purchases
  • Photographs or testimonials, only with your explicit consent

You may provide this information when you:

  • Create an account or sign up for a membership
  • Book an appointment, redeem credits, or purchase a service or pack
  • Subscribe to our newsletter or marketing communications
  • Contact us via email, phone, web form, or in person
  • Participate in a promotion, survey, or giveaway
  • Complete intake, waiver, or consent forms

2. Biometric Information (BIPA Notice)

HEAL operates a STYKU 3D Body Scan, which collects biometric information within the meaning of the Illinois Biometric Information Privacy Act (740 ILCS 14, "BIPA"). This may include 3D body geometry, body measurements, body composition, and related identifiers derived from the scan.

  • Purpose: STYKU biometric data is collected solely to provide you with body composition reporting, progress tracking, and personalized wellness recommendations associated with your HEAL membership or service.
  • Consent: HEAL will only collect, capture, or store your STYKU biometric data after providing you with a written BIPA disclosure and obtaining your signed, written consent. You may decline the STYKU scan at any time without affecting your access to other HEAL services.
  • Disclosure: HEAL will not sell, lease, trade, or otherwise profit from your biometric information. HEAL will not disclose your biometric information to any third party except (i) with your separate written consent, (ii) to complete a financial transaction you have authorized, or (iii) as required by law or valid legal process.
  • Storage & Protection: Biometric data is stored using reasonable industry-standard safeguards, including access controls and encryption where applicable.
  • Retention & Destruction: HEAL will permanently destroy your biometric information when the initial purpose for collection has been satisfied or within three (3) years of your last interaction with HEAL, whichever occurs first, in accordance with BIPA.

3. How We Use Your Personal Information

We use the information we collect to:

  • Process transactions, memberships, credit purchases, and bookings
  • Manage your account, credit balance, and pack expiration
  • Provide and improve our services and personalize recommendations
  • Communicate appointment confirmations, reminders, intake reminders, billing notices, and other service-related messages
  • Send marketing emails, newsletters, and special offers (only if you have opted in; you can unsubscribe at any time)
  • Maintain safety, prevent fraud, and enforce our policies
  • Comply with legal obligations and respond to lawful requests

4. SMS / Text Messages

If you opt in to SMS, you will receive transactional and/or marketing messages from HEAL Wellness + Longevity. Message frequency varies. Message and data rates may apply. Reply STOP to opt out and HELP for help. See our Terms of Service for the full SMS Program Description.

5. Aggregate & Non-Personal Information

We may collect non-personal statistical information about how visitors use our website (pages visited, time spent, links clicked, device type, browser, operating system, IP address, general geographic location). This information is collected through cookies and similar technologies and is used in aggregate. It does not personally identify you.

6. Cookies & Tracking Technologies

We use cookies and similar tracking technologies to:

  • Recognize returning visitors and remember preferences
  • Improve site functionality and personalization
  • Analyze traffic patterns and optimize performance

You can adjust your browser settings to block or delete cookies. Doing so may impact certain features of our site.

7. Third-Party Services & Data Sharing

We work with trusted third-party providers to operate our business. Each provider receives only the data needed to perform its function and is bound by industry-standard security and privacy practices.

  • Momence — Online registration, membership management, scheduling, credit ledger, and service bookings. Personal data (excluding full card numbers) is stored on Momence's secure platform. Card data is processed through Momence's PCI-compliant payment gateway.
  • Stripe (or Momence's payment processor) — PCI-compliant payment processing. HEAL does not store full card numbers.
  • STYKU — 3D body scanning vendor whose hardware and software capture biometric data, governed by Section 2 above and HEAL's BIPA Policy.
  • Email marketing platform — For promotional and newsletter emails to subscribers who have opted in.
  • Website analytics — To understand site usage and improve performance.
  • SMS provider — For text-message delivery if you have opted in.

Asher Med LLC — Separate Medical Entity

Medical services available on site (peptides, hormone optimization, GLP-1 weight-management programs, telehealth consultations, and related care) are provided by Asher Med LLC, an independent, separately licensed medical entity. Asher Med maintains its own patient records, privacy practices, and compliance obligations (including HIPAA where applicable). HEAL does not access, store, or process Asher Med patient health information. Asher Med will provide you with its own privacy and consent forms before providing any medical services.

Acupuncture — Independent Provider

Acupuncture services on site are provided by Pearl Lasky, L.Ac., an independent licensed acupuncturist. Pearl Lasky maintains her own patient records and is solely responsible for her clinical care and privacy practices.

8. Security Measures

HEAL takes reasonable and appropriate steps to protect your personal information from unauthorized access, alteration, disclosure, or destruction, including:

  • SSL/TLS encryption for online transactions and account access
  • Access controls limiting staff access to information they need
  • PCI-DSS-compliant payment processing through our payment gateway
  • Reasonable physical safeguards at our facility for any paper or local records

No system is 100% secure. By using our site and services, you acknowledge the inherent risks of data transmission over the internet. If you create an account, you are responsible for keeping your password secure.

9. Your Choices & Rights

You may:

  • Access or update your account information through your Momence account or by contacting us
  • Opt out of marketing emails by clicking unsubscribe in any marketing email or contacting us
  • Opt out of SMS by replying STOP to any text message from us
  • Revoke BIPA consent for STYKU biometric collection at any time by emailing info@heal4wellness.com
  • Request deletion of your account information, subject to records we are required to retain by law

Transactional and service-related messages (appointment confirmations, billing notices, safety information) may still be sent even after opting out of marketing.

10. Age of Consent

HEAL's services are intended for adults. By using our website and services, you confirm you are at least the age of majority in your state. Anyone aged 13–17 may use HEAL services only with verified parental/guardian consent and as outlined in our facility rules. No one under 13 may use HEAL facilities. HEAL does not knowingly collect personal information online from minors without parental consent. If you believe a minor has provided us with their information, please contact us so we can take appropriate action.

11. Governing Law

This Privacy Policy is governed by the laws of the State of Illinois, U.S.A. HEAL makes no representation that this Policy complies with the laws of other jurisdictions. If you are accessing our site from outside the United States, you do so at your own risk and are responsible for compliance with local laws. By using our site, you consent to the transfer and use of your information in accordance with this Privacy Policy.

12. Updates to This Policy

HEAL may modify this Privacy Policy at any time. Updates will be posted on this page with a revised effective date. If changes are significant, we may notify you by email or a website notice. Continued use of our site or services after updates constitutes acceptance of the revised policy.
